I think the OCG is off, but I'm not sure how to word it better, so maybe I'm wrong on that. Good effect, good amount for the payment, sort of good picture; I'd give it an 8/10. Just leave it at, "When this card is destroyed by a Trap card, you can pay 1000 Life Points to Special Summon it in face-up Defense Position." As for the OCG, if it's basically a tweeked version of Revival Jam's, it should be correct as you have it. That's what I usually do when I make a card similar to one that already exists, at least...
