Dark Wisper with OCG I believe to be correct:

Once per turn, during your Standby Phase, you can pay 500 Life Points to Special Summon 1 Level 4 DARK monster from your Deck in face-up Attack Position. Your Deck is then shuffled.

 

Not sure on the last sentence :/ Veteran help?

 

As a general rule of thumb:

• Attack/Defence Position is capped
  
• Deck/Graveyard is capped
  
• Attribute is written in caps on it's own (LIGHT)
  
• Type is written as {Type}-Type (Warrior-Type)
  
• Life Points is capped
  
• Normal/Special/Tribute/Fusion/Synchro Summon is capped
