How do you make card templates?

Depends on what type of card template. If you want to make templates of card types that already exist, i.e. the real cards, then you find a card of the type you want to make a template for (I use yugioh.wikia.com) and use Photoshop, GIMP etc. to copy clean parts, paste, blur, smudge etc. over the names, effects, etc. to make it look like an empty card/template.

For custom templates, use your imagination... I've never made any custom templates, so I can't help you there.

But for both, try looking at other member's templates to see how they made them, and start from there.
