Okay. I don't know where to start here, but I'll just go along and fix mistakes as I see them.

 

1. The Set ID - HES-UGLY? That's kinda ridiculous. The first part of the Set ID should be EN(insert 3-digit number here)-(Insert abbreviation of set name here).

2. There's no type. Since the material is all spellcasters, make the type spellcaster...?

3. Use capitals in your name. Most of your grammatical errors aren't even OCG (Official Card Grammar) errors, they're normal English errors. You should have learned that in school.

4. Okay. Here's the hard part for me. I've noticed a lot of mistakes in the effect, and when making a card, there is a proper way of saying everything. This is the proper way to say what you were trying to say:

 

"Right Arm of the Forbidden One" + "Left Arm of the Forbidden One" + "Right Leg of the Forbidden One" + "Left Leg of the Forbidden One" + "Exodia the Forbidden One"

This card can not be Special Summoned except for Fusion Summon. This card is not affected by the effects of Spell, Trap, or monster cards.

 

So, you can copy and paste that into the generator.

5. The name is already being used in a real card. Use something other than "Exodia the Forbidden One".

6. The card is pretty much useless. I mean, sure, it's got 5000 ATK and DEF. But getting that material into your hand is hard enough - how many times have you seen someone win with Exodia - and chances are, unless some of them are on your field when you grab a Polymerization (which is insanely unlikely, considering how weak the material is), you win anyways, since they're in your hand.

7. Gee, where have I seen that picture before? Don't use pics that already belong to real cards.

 

So, you probably won't (knowing the members on this site), but, you should fix your card somehow to make whatever I just said false. Then, it'll be a relatively good card. My rating for you is:

 

Name: 0/10. No originality.

Level: 5/10. Too high, even for a power beast.

Picture: 0/10. Bad quality and already used on a real card.

Type choice: 0/10. You didn't even choose a type.

Creativity: 1/10. Bland and unoriginal.

OCG: 1/10. Errors are plentiful.

Balance: 2/10. Useless and underpowered.

Other: 3/10. Well, the Set ID is unprofessional, it would also look better if you added a Circulation (Limited Edition, 1st Edition, etc).

Overall: 1.5/10. Not a good job. Newb bonus gives +1 for a total of:
