Basically, your opponent calls a name. If you have that card in your deck, you have to put it on the top. If you don't, you can place any card on the top. Might be good for not well known decks. It's some good search power, but it can backfire.

What about Karakuri.dek?
"Say a name"
"Karakuri Ninja...mdl...."
"What's the number?"
"I don't remember, but it's the one that..."
"Nope, sorry."
"F*ck you and your numbers."

Yeah, that'd happen. Good for not known decks, like Nephthys Turbo.dek or something.

Whether this is ridiculously broken or not, I must say that asking your opponent his opinion of your own deck is exceedingly clever. I can appreciate that, even if I now lack the imagination to envisage problems that this card might pose.

The most obvious cards to call are "Mystical Space Typhoon" and "The Inquisitor". This card is most powerful on an early turn when your opponent doesn't know what your Deck is like.

You can't call the inquisitor, read the effect.

How did I not see that? Mystifying.

In that case, it's pure guesswork, I suppose. It's not very good if your opponent can guess correctly since they'll probably hit whatever they can play around. However, in a Deck full of uncommon situational cards, your opponent will probably miss the first guess and you'll be able to respond to whatever uncommon situation is on the field now.
