If you apply more than 1 effect, it resolved in sequence. You can only apply each effect of "Babylonian Index" once per turn.

@ If that monster is not destroyed by the above effect: Banish that monster

@ If that monster is not Banished by the above effect: make his/her controller Shuffle that monster to its owner Deck. If this effect resolved you gain 1000 LP
