I had an idea for this yesterday, and finally got it down on paper today. Here are the first few cards. I'm not positive of the wording on the first and second so any help would be most appreciated ^_^

 

Mask of Identity

Equip Spell

Equip only to a face-up monster. Once per turn, during your Main Phase, you can switch the target of this card to another face-up monster. As long as it remains face-up on the field, the equipped monster is treated as having the original name of the previous target of this card.

 

NOTES: It works like this: Say I equip it to my opponent's Jinzo (he's the first to be equipped, so his name stays the same as there are no previous targets to copy the name from), then next turn I equip it to my Dark Magician; Dark Magician will now be treated as having the name Jinzo. Then, if next turn I equip it to his Buster Blader, Buster Blader will be treated as having the name Dark Magician (the original name of the previous target) and Dark Magician will still be treated as having the name Jinzo (since he's still face-up on the field).

 

Mistaken Identity

Counter Trap

Activate when a monster you control is selected as an attack target. Switch the target to a monster on the field with the same original name as the selected target's name.

 

NOTES: Continuing from the above example, let's say my opponent then attacks my Dark Magician (Jinzo) with his Buster Blader (Dark Magician), and I activate Mistaken Identity: the target would be switched to his Jinzo since it has the same original name as my monster's current name, and his monster would be attacked instead of mine (in this case, a monster can attack another monster with the same owner).

 

Identity Swap

Normal Trap

Select 1 "Mask of Identity" you control and switch it to another correct target.

 

NOTES: Just some support for Mask of Identity to help the name swapping craziness :P

That's not an error, it has to say "original name". Think of it like "original ATK" vs. "ATK": they're different and unique. In this case, "original name" means the name printed on the card, regardless of any name it's being treated as having currently.
