Destroy the Summoned monster and remove it from play, and deal damage to your opponent's Life Points equal to the Level of the destroyed monster x500.' Or something like that. However, I can't help but feel that this trap is suitably . . . broken. There's a reason that Trap Hole and friends are Spell Speed 2 (i.e., not Counter Traps), my friend, and for another thing if this thing ever were made real, it would either be Limited, Banned, or (as doth follow Banning) both. Destroys a monster? Check. Removes it from play? Check. Doesn't care about ATK or DEF when doing so? Check. Covers 90% of all played monsters? Check. Can't be chained to by Stardust? Check. Burns for a potentially-heavy sum (e.g., Stardust would equal 4000 damage)? Check. You might want to tone the burn down significantly, or otherwise make the requisite for the portal a little bit . . . less free-range. Perhaps add a ceiling as well as a lower limit.
