(Yes/No)If yes, what kind?Serial #:Set ID:Gold? (Yes/No) Application For Regular Holo:Name:Trap/Spell/Monster? (Pick One)If Trap:What Type? (Continuous, Counter, etc.)Effect:Image:Set ID:Serial #:If Spell:What Type? (Equip, Quick-Play, etc.)Name:Effect:Image:Set ID:Serial #:If Monster:Name:Type(Zombie, Dragon, etc.) or you can make it up:Subtype (Effect, Tuner, Gemini, Etc.)Effect (Or Lore):Set ID:Serial #:Level:Edition:ATK/DEF: Application for RenderPic:What you want rendered: Note: All images MUST be 245" by 250" they won't fit in the card if it's bigger or smaller.
