this is how I worded the effects

Must be Fusion Summoned and cannot be Special Summoned by other ways. Once per turn, if this card is targeted for an attack: Double its ATK. At the end of the Battle Phase, destroy all monster(s) this card has attacked. When this card is destroyed by a card effect and sent to the Graveyard: Special Summon 1 "Elemental HERO" Fusion Monster from your Graveyard (ignoring the summoning conditions.)

Pretty sure it's "cannot be Special Summoned by other ways", but it's been a while since I've really looked at OCG. The rest of your fix looks good to me.
