I think the card is lackluster: depends on your opponent doing something, and summons from the hand so it actually isn't netting card advantage on its own. You can make it better by allowing it to Summon from the Deck as well, as a sort of reactive Transmodify. It can then be further improved my making it easier and/or faster to activate; you could make it a Normal or Quick-Spell card, or make it activate on your own removal effects for combos.

I guess this would work okay for Decks that don't really Xyz spam often or suppose high Level boss Decks (only one coming to mind right now is Blue-Eyes floating into Dragon Spirit or something). I don't know; part of me thinks you can remove the Graveyard requirement, but then we have Pendulum spam that would need to be addressed.

 

Suppose it can help with emergency on-trigger effects in certain Decks, but not really seeing much out of it. (It's an upgrade over your original version though)

When a monster you control is sent to the Graveyard by an opponent's card (by battle or effect): Special Summon 1 monster from your hand or Deck with the same Attribute, Level (or equal to that monster's Rank) and Type as that monster, but with a different name.

 

Working with the P-effect of Creation/Xiangsheng Magician to some extent; may not be (precisely) correct wording, but should be close if you want to incorporate Xyz Monsters.

I think a better wording would be:

If a monster you control is sent to the Graveyard by an opponent's card (by battle or card effect): Special Summon from your hand or Deck, 1 monster with the same Attribute and Type as that monster, and a Level equal to that monster's Level/Rank, but with a different name.

 

I'm still pondering if it should be further improved by making it activate on your own removal effects, but then it could be used to bring lock monsters such as Spell Canceller, Rai-Oh, etc. at the end of a combo, making it abusable. Still, I think it can get away with the the "and send to the Graveyard" requirement"; I doubt it would cause much trouble along Pendulums.
