I like this.  I think the only thing I'd suggest is actually removing the HOPT.

1. There is no benefit to playing multiples of the card in one turn (other than for reducing your LP).
2. Coin toss decks are so weak that even the minor buff (playing a second if the first is negated) would be extremely helpful.

For this one, I am kinda worried if someone use this card only to lower their LP. But it's a card that makes you lose LP for nothing, so ok, let me remove the HOPT Shackle.

Sometimes I confuse with the difference between "You can only use this effect of ..... once per turn" and "You can only activate 1 .... per turn".

No worries.  I have to keep looking that up myself.  If a card with "use this effect..." has its activation negated, you cannot activate another copy.  But if it has "activate 1..." you can activate a new copy.  There are other general conventions for which to use, but that's a spiel for another day.
